    Lost your White Card? We can issue a replacement if you trained in Queensland and can provide your Statement of Attainment.

    How do I apply for a replacement card?

    Quick Reference

    QLD students: We can issue you a replacement White Card if you did your original training with Blue Dog Training OR if you have a Statement of Attainment or your USI VET Transcript as evidence from another Registered Training Organisation (RTO) in Queensland.

    WA students: Replacement cards can only be issued by the original RTO. If they have permanently closed, please contact our head office on (07) 3331 6001 for further instruction.

    TAS students: Replacement cards cannot be issued. Contact Service Tasmania on 1300 366 322 to enquire about replacing your White Card.

    Lost your QLD Construction Blue Card?

    If you have lost your previously issued Blue Card, it cannot be replaced. You will need to complete a White Card Course (CPCWHS1001 Prepare to work safely in the construction industry) and be issued with a new White Card. You can book a QLD White Card Course here.
